NOTICE TO ALL PATIENTS

I am delighted to advise you that with effect from 1st July 2020 I have been joined by two further doctors to provide medical services to you. These two doctors will become Partners of the Practice with me. The new doctors are Dr Wasim who was until recently a salaried GP working at the Practice and Dr Nazir, who has previously been at the Practice as a GP Registrar and Locum GP.

NOTICE TO PATIENTS

OUR TEMPORARY phone line for patient use which is 0161 628 5604 will be withdrawn AFTER Friday 10th July 2020.

APPOINTMENT SUSPENSION

Due to risk of Coronavirus infection. Following local and national guidance we are no longer able to book routine appointments. If you have a routine appointment booked with a GP, this will now be a TELEPHONE CONSULTATION. Please DO NOT come to the Practice unless you are asked to do so by a Doctor. Routine and emergency telephone appointments are available everyday but not pre bookable

Our doors are closed for the foreseeable future. Please only attend the surgery if asked by a GP/Nurse. Samples & correspondence can be posted through the letter box. Our phone lines are still open

Thank you - We are here to help and support you.

Non-NHS Services

There are occasions when you may require a service from your doctor which does not come within the terms of the NHS. Your doctor is under no obligation to provide this service and is entitled to charge a fee.

All items for insurance claims, mortgage protection, and holiday insurance for a doctor to fill in are to be left with the Receptionist (NOT the doctor) making sure you have completed and signed the form.

The doctor often has to state details you have given are correct and if the form is incomplete this can lead to delays in your form being finalised.

Confirming claimant’s statements and actions can have legal consequences. You may be asked to make an appointment to see a doctor if further information is required.

The practice charges the agreed British Medical Association fees for filling in forms.

There may be a charge for other services such as, private medical certificates, completing passport forms, pre-employment certificates etc. Please ask at reception for details.
